The entrance is a worn stone portal partially submerged by the swamp. It sits right in the Pool of Tears, near coordinates 75, 45 on your map

Approach via the shallow eastern or northern shores of the Pool of Tears. You’ll see steps leading down—head through the portal into the instance.
🗺️ Why “Sunken Temple”?
Lore-wise, Ysera and the green dragonflight sank the temple centuries ago; it’s now half-submerged, hence its classic designation “Sunken Temple”
